Student Registration

New Student Enrollment Checklist

Required Documents:  The following documents are required to enroll your student and should be furnished at the time of enrollment of your student along with a completed student enrollment packet at the Welcome and Enrollment Center.

Original Birth Certificate

Original Social Security Card

Immunization Records

Proof of Residency (contract on home OR lease agreement OR 2 utility bills, gas, water, electric or property tax only)

Emergency contacts and phone numbers

Previous school address, phone and fax numbers

Special Services (IEP or Speech)

Mo Safe Schools Attachment (need previous school's name, address, phone and fax numbers)

Health Information Form

 

Pre K - Early Childhood Center

This preschool serves children who qualify to attend based upon screening and evaluation information.  It is funded through state and federal funds for children with delays and/or disablities.

 

All New LPS Students

(Grades K-12)

Visit or contact the Enrollment Center at 736-5310 for an enrollment packet or print packet from the website.The Enrollment Center is located at the north end of the District Administration Center, 650 Conistor St., under the blue awning.  You can pick and drop off Enrollment packets from 7:00 am to 4:00 pm Monday - Friday.

Hours:

Monday - Thursday 7:00 am - 4:00 pm 

Friday's 7:00 am - 3:00 pm

Closed Thursday's from 11:45 am - 12:45 pm

 

Step 1

Parent receives, completes and returns Liberty Public Schools Enrollment Packet and provides required documents (listed above).

The Enrollment Center will contact your child's previous school for all academic, discipline, psychological, and special services testing, and IEP, test and health records and all information pertinent to the educational program of the student.  Elementary School secretary will call parent when all information is in hand to confirm start date.  Secondary counseling secretary will call parent when all information is in hand, an appointment will be set to meet with a school counselor.

 

Step 2

If eligible for bus transportation, student will need to wait 48 hours after school enrollment is complete to begin riding the bus. This is to accommodate the transportation department planning. Their phone number is 736-5360. This means the student may need to have transportation provided by the parent the first two days of their school attendance.

 

Step 3

Student should bring general school supplies to the first day of school.

Parent Items:

  • Pay fees, purchase agenda, and pick up school calendar in front office
  • Put money on lunch account if desired
  • Speak with nurse & turn in medications if needed

Student Items:

  • Secondary-level students review schedule of classes with counselor
  • Take school tour with a Student Ambassador
  • Begin classes with general supplies of a pen and spiral notebook
