Elementary Enrollment Process
- Complete enrollment forms at the district's Welcome Center, 650 Conistor and provide required documentation.
ENROLLMENT HOURS:
Monday - Friday 7:00 am - 4:00 pm
Summer Hours beginning June 4 - July 20, 2012
Monday - Thursday 7:00 am - 5:30 pm
Friday - CLOSED
- Forms will include:
- Basic Demographic Information
- Health Information
- Release of Information
- Missouri Safe Schools Forms

Information needed for enrollment:
Social security number, birth certificate, immunization record, and proof of residency
- Proof of residency is any of these three items:
*Two different utility bills (gas, water or electric only)
*Lease agreement
*New contract for purchase of home
Proof of residency must be in one or both parent’s name(s). Under most circumstances, student may attend within 48-72 hours provided the enrollment forms are complete and the information above is provided. Students may not attend the same day they are enrolled.
