Middle & Junior High Enrollment Process

Middle & Junior High Enrollment Process

Middle School - 6th and 7th

Junior High - 8th and 9th

Obtain and complete enrollment forms at the district's Enrollment Center, 650 Conistor St. and provide required documentation.

 

Step 1

Parent receives, completes, and returns Liberty Public Schools Enrollment Packet and provides required documentation. Parent may also include any student records they have brought to Liberty themselves.

 

Required Documentation:

 

*Basic Demographic Information

*Social Security Card

*Immunization records

*Missouri Safe Schools Form - Release of information (in packet) 

*Proof of Residency (any of these three items)

     *Two different utility bills in parents name(s) (gas, water or electric only)

     *Lease Agreement

     *New contract for purchase of home

     *Contact Information (name, address, fax #) from most recently attended school.

Parent should let the counselor or secretary know if student desires to enroll in band, orchestra, or vocal music.

 

Step 2

The Enrollment Center faxes previous school for records. School records include grades, test scores, attendance records, discipline records, and immunization records. If appropriate, we require the current IEP or 504 Plan for those receiving special services.

Items required to be in hand BEFORE the student starts school:

  • Immunization records must be current. School records must be faxed or mailed DIRECTLY from previous school; if parent hand delivers these papers, the counselor will call and get verbal verification of records
  • IEP or 504 Plan (Special Services info) if applicable.
     

Counselor will call parent when all information is in hand. Student typically start within 48-72 hours of receipt of enrollment packet and all required documents.

Step 3

Parent will contact the transportation department at 816-736-5360 to get bus information if needed. This step can be done the day after the enrollment paperwork has been turned in.

 

Step 4

Parent brings the student to school on his/her first day and works with the counselor to finalize enrollment process. Student should bring general school supplies. If the parent cannot attend, they may finalize these items ahead of time.

Parent Items:

  • Pay fees, purchase agenda, and pick up school calendar in front office
  • .
     
  • Put money on lunch account if desired.

  • Speak with nurse & turn in medications if needed.

  • Get brochure for ParentCONNECT and teacher contact information.

Student Items:

  • Review schedule of classes with counselor
  • .
    Take school tour with a Student Ambassador.
    Begin classes.
