Summer Hours beginning June 4 - July 20, 2012

The following information is required and must be on file with the Liberty Senior High School Guidance Office before an appointment with a student's school counselor can be made. Basic Demographic Information

Proof of Residency (any of these three items)

     *Two different utility bills

          (gas, water or electric only) Must be in one or both parent's name(s)

     *Lease agreement

     *New contract for purchase of home

     *Contact Information (name, address, fax #) from most recently attended school.
